If you've never created a form before, the easiest way to start is by asking the Form Page Wizard to do it.

Like other wizards in FrontPage, the Form Page Wizard asks a series of questions to determine what you'd like to add to your Web:

• What questions do you want to ask?

• What kind of answers are acceptable?

• How should the questions and answers be formatted on the Web page?

• What should happen to the answers afterward?

When all of this has been determined, the wizard will create a new page in the current Web and add the form to it. You can either use this page or transfer it to another page using copy and paste.
